Terms & Conditions for Employment & Volunteer Applicants
By submitting an application for employment, volunteer service, or independent contractor opportunities with PFMDINC Services, you agree to the following Terms & Conditions:
Accuracy of Information
You certify that all information provided in your application, resume, and supporting documents is truthful, complete, and accurate. PFMDINC Services reserves the right to deny or terminate employment or volunteer placement if false information is discovered.
Background Screening
All applicants may be required to undergo a Level 2 Background Check, reference check, identity verification, drug screening, and credentials verification as required by Florida law, AHCA guidelines, Medicaid, or organizational policy.
You agree to provide all necessary documents and consent forms for screening.
No Guaranteed Employment
Submitting an application does not guarantee employment, placement, or interview. PFMDINC Services maintains the right to accept or reject any applicant at its sole discretion.
At-Will Employment
Employment with PFMDINC Services (where applicable) is at-will, meaning either the organization or the employee may terminate the relationship at any time, with or without cause, consistent with applicable law.
Confidentiality & Privacy
During the application process, you may receive or access confidential information. You agree not to disclose or misuse any sensitive information related to clients, staff, or company operations.
PFMDINC Services protects applicant information under HIPAA (if applicable), state regulations, and internal privacy practices.
Compliance With Policies
If hired or accepted as a volunteer, you agree to follow all PFMDINC Services policies, procedures, codes of conduct, safety rules, and regulatory guidelines, including those required by:

Use of Applicant Data
PFMDINC Services may use the information you provide for:

Electronic Signatures & Communication
